1. I have read and understood, the Camp Oak Hills Retreat Policies, Doctrinal Statement. Mission Statement of Camp Oak Hills and Oak Hills Fellowship (which may be referred to collectively herein as “Oak Hills”).

2. The following are 9 values of the ministry of Camp Oak Hills.  Our desire is that groups requesting to use camp’s facilities/services share some of these values.  Please select at least 3 values from the following list, and briefly describe how you would implement them into your retreat.
