25.00 DEPOSIT / person, per week must accompany this form. The balance is due upon arrival. This fee is not transferable or refundable unless there is no room in the camp. Please allow 3-4 weeks for confirmation. After submitting this form you will be directed to the online payment page. If you are making multiple registration, you only need to pay once after submitting your final registration. You can also make your deposit with a check. Checks (payable to Camp Oak Hills) can be mailed to: Camp Oak Hills 1696 South Camp Road SW Bemidji, MN 56601.
Your registration provides Camp Oak Hills the authorization to use photos and videos of you and your registered group for promotional purposes.
Free Camp T-shirt
If registered by May 1st with a deposit, your camper will receive a free camp t-shirt at registration.

Scholarship Information
Scholarship assistance is available for campers whose families are financially unable to send their children to camp. To request a scholarship, first register your camper and pay the $25 deposit if possible. Then fill out the online scholarship request form on the "Forms and Downloads" page.
